Topo-guided LASER Treatment and Crosslinking for Keratoconus / Irregular Astigmatism

Medicine is made up of countless hours of theoretical research, technical design and practical application, which over years of improvement will inevitably result in a treatment that will potentially eliminate or reduce the negative impact of a disease. 

In this case, a young man with grade 2 keratoconus, causing irregular astigmatism and loss of quality of vision, was treated using a combination of two techniques:

1. Topo-guided PRK: Using excimer LASER and advanced mapping technology to accurately reshape the cornea according to the unique topography of the patient's cornea. PRK aims to improve visual acuity by reducing irregular astigmatism associated with keratoconus.

2. Crosslinking (CXL): A relatively new procedure that aims to strengthen the cornea, using the application of riboflavin solution followed by controlled exposure to UV-A light. The collagen fibers of the cornea are strengthened, improving their structural integrity and preventing the progression of the disease. 

The combination of the two techniques has shown promising results in safely and minimally invasive in improving the refractive error and quality of vision, as well as optimizing corneal strength. In this case, after a few weeks, there was very significant improvement in vision without correction and a marked reduction in refractive error.
